Complete the forms below for: *Food Allergies In the case of a milk allergy, we offer a nutritionally equivalent replacement of soy milk. *Medically Documented Celiac Disease or Gluten Intolerance If your child has celiac disease or gluten intolerance and we have received the above Special Diet Statement for your child with a physician's signature, your child is able to receive gluten-free foods. Our gluten-free menu is planned each month to resemble the normal lunch menu as closely as possible. *Disabilities or Medical Conditions The participant has a disability or medical condition that requires a special meal or dietary accommodation. *Lactose Intolerance In cases of lactose intolerance, we offer a nutritionally equivalent replacement of lactose-free milk. Juice is not an acceptable substitute.
